EMERGENCY CARE AUTHORIZATION:
In the event of illness or injury during grooming, I authorize, emergency veterinary
care for my pet if I am not immediately available.

PICK UP/ DROP OFF AGREEMENT:
I understand and agree to the pick up and drop off times. I will be sure to leave in a timely manner to make my booked appointment on time and communicate if there are any unexpected delays in dropping off along with picking up. I understand that I will be charged a late fee ($10) if I am more than 15 minutes late for drop off and/or pick up. I’m also aware the groomer will make reasonable efforts to adhere to the scheduled times, but may have slight variations due to unforeseen circumstances that will be 100% communicated with the client.

CANCELLATION AND RESCHEDULING POLICY:
I will make sure to communicate any rescheduling or cancellations at least 48 hours prior to the appointment and understand I will be charged 50% of the service amount if I do not notify before 48 hours.

RELEASE AND CONSENT:
I hereby consent to grooming services for my pet, and release the employees, and affiliates from any liability for accidents, injuries, or other incidents that may occur during or as a result of grooming.

MATTING:
I hereby authorize the grooming services for my pet, with the understanding that if my dog is matted, to be removed, it may require: Trimming/shaving, possible change in appearance, risk of skin irritation, and must continue routine maintenance. I also understand that there may be additional fees for dematting.

PUPPY AND SENIOR GROOMING:
I understand that grooming a puppy or senior dog may involve certain risks such as, stress or anxiety, Age-related health concerns that require more grooming time, sensitive skin, or behavioral reactions. I trust that groomer will take all necessary precaution to ensure my pets safety and well-being during the grooming service.

REFUSE SERVICE:
I understand that at any point during any grooming session, the groomer may refuse to service my dog for any reason they see fit, such as: uncontrollable aggression, serious stress and anxiety for the dog, anything that may cause both the groomer or dog to feel unsafe.

PHOTO CONSENT FORM:
I understand and consent to the taking of before and after photos of my pet during the grooming service. I understand that these photos may be used for documentation and marketing purposes.
